1. Does your organization have its own IRS determination letter that states it is exempt under section 501(c)3 of the Internal Revenue Code?

(The Trust makes grants only to tax-exempt organizations whose purposes are described in Section 501(c)(3) of the Internal Revenue Code that are either public charities described in Sections 509(a) or private operating foundations described in Section 4942(j)(3).

2. The Trust accepts requests only from charitable organizations located in and serving the metropolitan areas of Indianapolis, Indiana and Phoenix, Arizona, with the exception of environmental requests. Please select the geographical area below that your organization serves.

3. If your organization has received a previous grant from the Trust, have you completed the grant term and submitted a final evaluation report to the Trust?

(Before applying for another grant, you must have completed your grant term and a final evaluation report must be submitted to the Trust.)



4. Has your organization applied for a previous grant in the current calendar year?

(Requests for funding are limited to one request per organization per calendar year. This includes the Summer Youth Program Fund.)

5. If your organization has never received a grant from the Trust, have you communicated with a grants programs staff person about your proposed application?

(Organizations that have never received a Trust grant are required to speak with a grants programs staff person prior to submitting a preliminary proposal. The Trust strongly suggests that former grantees also contact a grants programs staff person prior to submitting a preliminary proposal.)

7. Are the majority of your board members independent? Independent members do not receive compensation by the organization nor are they related to anyone that is compensated by the organization.

(Your organization must have a minimum of three members on its governing board, and at least two-thirds of the members should be independent.)
